What is Happening? — Know Your Condition

Modern Medicines — Dose & Senior-Citizen Alerts

Medicine Class Standard Dose Key Side Effects
Tamsulosin
(Flomax, Urimax)
Alpha Blocker 0.4 mg once daily
After same meal daily
Dizziness, low BP on standing (orthostatic hypotension), retrograde ejaculation, nasal stuffiness
Alfuzosin
(Xatral, Alfoo)
Alpha Blocker 10 mg once daily
After evening meal
Dizziness, headache, fatigue, low BP; fewer sexual side effects than tamsulosin
Finasteride
(Proscar, Finpecia)
5-ARI 5 mg once daily
Takes 3–6 months for effect
Reduced libido, erectile dysfunction, reduced ejaculate volume. Lowers PSA — tell your doctor before PSA test
Dutasteride
(Avodart, Dutas)
5-ARI 0.5 mg once daily
Can be taken with or without food
Similar to finasteride; breast tenderness (gynecomastia) in some men
Solifenacin
(Vesicare)
Antimuscarinic 5 mg once daily
(may increase to 10 mg)
Dry mouth, constipation, blurred vision, urinary retention risk — use with caution in elderly
Mirabegron
(Betmiga, Myrbetriq)
Beta-3 Agonist 25 mg once daily
(may increase to 50 mg)
Raised blood pressure, urinary tract infection, headache. Safer than antimuscarinics for elderly cognition
⚠️ Special Alert for Senior Citizens (Age 60+) Risk of Falls: Alpha blockers (tamsulosin, alfuzosin) cause orthostatic hypotension — sudden blood pressure drop on standing. This raises fall risk in elderly. Always rise slowly from bed or chair.

Cognitive Risk: Antimuscarinics (solifenacin, oxybutynin) can worsen memory and cause confusion in seniors — discuss alternatives with your doctor.

PSA Caution: Finasteride & dutasteride lower PSA by ~50%. Always inform your doctor you are on these before a prostate cancer screening test.

Drug Interactions: If you take blood pressure medicines, alpha blockers can dangerously lower BP further. Never self-medicate. Consult your physician.

Daily Lifestyle — Do's & Don'ts for Seniors

  • Drink 6–8 glasses of water daily — but reduce fluid intake after 7 PM to limit nocturia.
  • Practice Kegel exercises (pelvic floor exercises) daily to strengthen bladder control.
  • Vajrasana (sitting on heels posture) for 10 minutes after meals — stimulates urinary health in Ayurveda.
  • Eat a diet rich in vegetables, pumpkin seeds, tomatoes, broccoli, and pomegranate.
  • Avoid tea, coffee, and alcohol — these irritate the bladder and worsen urgency.
  • Avoid spicy, fried foods and cold drinks that aggravate Vata-Kapha imbalance.
  • Never ignore the urge to urinate — delayed voiding stretches the bladder.
  • Get PSA test + ultrasound (post-void residual) checked annually after age 60.
